Healthcare has spent the last decade becoming digital.
Patient records and scheduling moved online. Data became easier to capture and share. Electronic health records and practice management systems replaced many of the paper-based processes that healthcare organisations had relied on for decades.
This was a necessary step forward. Without these foundations, much of the innovation healthcare is exploring today wouldn’t be possible.
The issue now is, having the information available is not the same as being able to act on it.
A clinician has access to a patient’s history, previous consultations, test results, treatment plans. A healthcare organisation has dashboards that show demand and operational performance.
But what happens next?
Who follows up with the patient? Who schedules the next appointment, or makes sure a referral progresses? And ultimately, how does the organisation know that the right action has happened at the right time?
These questions are what’s shaping the next phase of healthcare transformation.
The first era of healthcare technology was built around systems of record
Most healthcare technology introduced over the last decade has been designed to solve a fundamental challenge: creating a reliable record of what has happened.
Systems of record give organisations a place to store and access important information. This helps teams across the board: clinicians understanding a patient’s history; administrative teams managing appointments and billing.
Essentially, they create a shared source of truth across an organisation, which until recently has been hugely valuable: healthcare is notoriously complex, and without accurate information it becomes almost impossible to deliver safe, effective care at scale.
But information doesn’t deliver care. Let’s think about a patient who visits a sports medicine clinic with persistent knee pain. Following the consultation, the clinician recommends an MRI scan, a follow-up appointment to review the results and a course of physiotherapy if surgery isn't needed.
From the perspective of the patient record, everything has happened correctly. But the patient journey is only just starting. The scan needs to be booked, the results need reviewing, the follow-up appointment needs arranging and the physiotherapist needs the right clinical information at the right time.
Each organisation will be delivering good care within its own part of the pathway, but the challenge is making sure those individual parts connect. Every additional handoff creates another opportunity for delay, duplication or miscommunication, which are typically associated with poorer patient outcomes and higher healthcare costs.
This distinction is becoming increasingly important as healthcare pathways become more complex.
Healthcare’s next challenge is consistent execution
Most healthcare organisations do have a clear understanding of what should happen next, but the difficulty is ensuring that the right action happens every time, especially when that journey involves multiple people and departments or even other organisations.
As patients make use of Right to Choose, this could complicate things even further. A patient accessing mental healthcare through a pathway that involves both private and NHS services might receive an ADHD assessment from one provider, begin treatment with a specialist and later need ongoing support from another part of the healthcare system.
Operationally, the pathway now spans clinicians, administrators, pharmacies, GPs and different organisations; every transition creates an opportunity for friction.
This becomes even more difficult as organisations scale. A small practice might be able to manage complexity through individual knowledge and communication between a close-knit team. A larger healthcare group with multiple locations, specialities, hundreds of clinicians and thousands of patients needs more reliable infrastructure to ensure pathways are followed consistently. That’s where the limitations of traditional systems start to become more visible.
Moving from information storage to enabling action
The next generation of healthcare technology needs to help organisations act on what they know. This is the move from a system of record to a system of action.
Rather than just storing information, a system of action uses that information to trigger, coordinate and monitor the work that needs to happen next.
This doesn’t remove clinical judgement or replace the expertise of healthcare professionals. Instead, it means reducing the operational effort required to keep complex pathways moving.
We know that AI tools are already creating significant value in the healthcare industry. For example, an AI scribe can produce consultation notes before a patient has even left the room. That’s a huge improvement for clinicians. But the value of the speed and efficiency created by scribes completely depends on what happens afterwards:
Does the consultation trigger the appropriate follow-up?
Does the patient receive the right communication?
Does the referral move forward?
Does the care team know what action is required?
The consultation is only one moment in the wider patient journey. The admin work doesn't stop when the consultation ends – in fact, around a third of a community-based clinician's time is estimated to be spent on administration and patient coordination. The technology surrounding the tools needs to help carry that momentum forward.
Why healthcare needs more configurable tech
Healthcare has historically struggled with automation because its care pathways aren’t standardised in the same way other industries might be.

A fertility clinic might need patients to complete questionnaires before every appointment, while an orthopaedic service may need imaging reviewed before surgery is booked.
